WHAT ARE THE MK DEAL PRIORITIES?

IMPROVING SYSTEM FLOW

Improving System Flow (ISF) focuses on urgent and emergency care services for older and/or frail and/or complex service users, in terms of facilitating safe hospital discharge and avoiding preventable admission in the first instance. The project will look at improving the current services in place - checking for gaps, duplications, IT system issues, and what works well and/or not so well, ultimately moving towards a more integrated approach.

TACKLING OBESITY

Tackling Obesity is focused on helping people lose weight and maintain a healthy weight through easily accessible weight management programmes, use of technology, pharmacological therapies, and education/prevention work.

The Tackling Obesity Priority has three key themes:

â–ª Theme 1 focuses on increasing referrals, access to and engagement with weight management services.

â–ª Theme 2 focuses on innovation and upscaling.

â–ª Theme 3 focuses on shaping the environment.

PEOPLE WITH COMPLEX NEEDS

Work on this priority has not yet started.

Once there is more information, this area will be updated.

BLETCHLEY PATHFINDER

A neighbourhood model is being developed across Milton Keynes where health, social care and other services such as the police, schools and voluntary and community groups work together to improve access, health prevention support and personalised care to us in our local communities. The first step to develop this model is piloting the first MK neighbourhood – this is called the Bletchley Pathfinder.

Milton Keynes Health and Care Partnership asked Healthwatch Milton Keynes to speak to people living in Bletchley to find out how they currently experience care, and what they would like, and need, to see from services to help them stay well for longer. Please click here to see what Healthwatch were told.

SHARE YOUR INSIGHT

The VCSE has a number of representatives on key health, care and community partnerships/committees and in order for representatives to share the views of the sector, they need to hear from you.

Throughout the year, The Voluntary Sector Alliance will hold 'meet the reps' sessions to gather your insight around upcoming partnership/committee topics.

These opportunities will be shared on our website as well as via the Community Action: MK newsletter.

Additionally, information can be shared anytime via our google form.
